New Family Law Court Forms 2018

The Ministry of Justice has introduced five new court forms for the family court in England & Wales in 2018. New forms FM1, Form B, Form A1, Form A and C100 have come into effect on 8 January 2018. Links to each form are set out below.

The old forms will no longer be accepted by the courts. All completed applications using the old forms will be returned to the applicant. The changes have been made following the recent changes to the family court rules on domestic abuse.

The amended family forms are:

Form FM1 (Family Mediation Information and Assessment Meeting (MIAM));

Form A (Notice of [intention to proceed with] an application for a financial order);

Form A1 (Notice of [intention to proceed with] an application for a financial remedy (other than a financial order));

Form B (Notice of an application to consider the financial position of the Respondent after the divorce/dissolution); and

Form C100 (Application under s 8 of the Children Act 1989 for a child arrangements, prohibited steps, specific issue order or to vary or discharge or ask permission to make a s 8 order).
